A mental health assessment performed by a qualified medical health professional determines if there is a need for admission to the Providence DePaul Center.

An assessment is comprised of in-depth questions that are personal in nature and should be answered as honestly and completely as possible. It includes a general medical and psychiatric history. Questions will also be asked in regards to current and past drug use and current stressors, situations and events, as well as other sensitive matters. An assessment and its contents are confidential regardless of admission. Our staff members are sensitive to all situations and any information requested is confidential.

An assessment is not for the purpose of immediate psychiatric treatment (including prescription medication or refills), to provide emergency counseling sessions or to receive treatment from a psychiatrist.

If it is determined that admission to the Providence DePaul Center is not required, a referral to other resources will be provided. An assessment does not guarantee admission to the Providence DePaul Center.

Hours for Walk-In Assessment

  • 8:00 a.m. – 8:00 p.m. 7 days a week
  • After hours: Present to ER at Providence Hospital
