We Appreciate Our Referring Physicians
Referring physicians play at vital role in the success of our patients at Providence Surgical Weight Loss Center. Patients being referred for weight loss surgery will need to have their medical records, documentation of a medically supervised diet and/or exercise program with monthly weigh-in’s, and other pertinent clinical information sent to Providence Surgical Weight Loss Center for evaluation. Information can be faxed to (254) 751-4557.
Because we believe in comprehensive care through a coordinated team approach, we will keep you apprised of your patient’s progress throughout our program.
